Job Description

As a Bilingual Information & Referral Representative , you will support the assigned team in ensuring patients receive prompt, effective customer service and provide accurate, comprehensive health and community information across the Champlain region.

Primary Responsibilities

  • Answer and respond to all incoming calls in a timely, professional and culturally sensitive manner in the caller’s official language of choice, determining the nature of the request and identifying issues and concerns.
  • Provide information and referral options to callers and, when appropriate, link to related services and resources such as Home and Community Care Support Services Champlain Intake Care Coordinators.
  • Promote, encourage and guide use of electronic navigation and self‑service tools (e.g., Champlainhealthline.ca).
  • Track I&R call information in accordance with monitoring requirements.
