As a Care Partner, you’ll guide customers through their care journey with compassion. Working closely with families, clinical teams, and external providers, you’ll develop holistic health plans, coordinate services, and ensure customers receive safe, person-centred care that enables them to age well at home or in a place of their choosing.
This is a hybrid opportunity with the option to work 2 days from home and 3 days from the Clayton Victorian Branch. You will be supporting customers virtually, over the phone within this role.
