Provides case management assistance to a trans-disciplinary team who service complex clients and adults with acquired brain injuries. Coordinates services for clients and families. Supports the team with completing various contractual, financial and administrative duties. Responsibilities Coordinates contract, financial, and administrative aspects of client services and case management under the direction of managers and clinical staff. Supports clients accessing services or managing urgent issues in the absence of the case manager and ensures that the case manager is then kept informed; reviews files to obtain historical information and reviews materials such as policy and procedures to assist with various case management issues. Researches community resources and identifies information that will be of assistance in the development and implementation of client services and case management plans.
