Enhance community health as a Case Manager Registered Nurse with Assisted Living Alberta in Consort. Build lasting relationships while directing Home Care services and engaging with clients.
As a Registered Nurse Case Manager, you will report to the Care Manager while managing client care within the Home Care program. Utilize your assessment, organizational, and leadership skills to identify client needs and coordinate resources through an interdisciplinary team. Effective interaction with clients and families is key for developing care plans and facilitating education.
Key Responsibilities:
• Direct care for clients under the Home Care program
• Facilitate care planning with clients and families
• Ensure ongoing modification of care services
• Provide wound care and manage home therapies
• Collaborate with the interdisciplinary team for referrals
Requirements:
• Accredited nursing education program completed
• Active CRNA registration or eligible for it
