The Clinical Consultant Pharmacist is responsible for maintaining continuity of care for clientele through collaboration between onsite physicians, nursing staff, and other health providers at long‑term care, supportive living, and other congregate living communities.

Role Accountabilities

• Act as a liaison between long‑term care homes and retirement communities and CareRx Pharmacy to ensure optimal resident care, pharmacy services, and clinical support
