About You

You are a compassionate and highly organized professional with a strong background in supporting children, youth, and families navigating complex and/or multiple special needs. With a degree in a related health or social services field and at least three years of direct experience, you bring a deep understanding of child and youth mental health, developmental challenges, and the systems that support them.

You excel in building collaborative relationships—with families, caregivers, and community service providers—to develop and implement Coordinated Care Plans that are family-centered, goal-driven, and reflective of individual needs. You are skilled at facilitating service coordination meetings, navigating funding and referral processes, and advocating for appropriate resources to enhance quality of care and support.
