Summary:
Performs a combination of administrative and data management duties to support grant-related programs. Provides day-to-day administrative and care management support by maintaining organized records, assisting with follow-up activities, and facilitating communication related to patient care within the pediatric primary care environment. Provides coverage and overall support to the care coordination team and program leadership, assisting with caseload management, monitoring referral status, and helping to ensure timely completion of tasks and program requirements.
