Support the aging community as a part-time Occupational Therapist with Senior Persons Living Connected. Focus on delivering tailored assessments and interventions for seniors in the GAIN Community Team.
This part-time, on-site position involves working closely with older adults to address their emotional and physical needs through comprehensive assessments. You will help create effective strategies to improve seniors' functional abilities while working with a collaborative healthcare team.
Key Responsibilities:
• Engage in Comprehensive Geriatric Assessments (CGA)
• Conduct clinical triage for new client referrals
• Gather and log assessment data thoroughly
• Provide care to enhance daily activities for seniors
• Collaborate on personalized care plans with team members
Requirements:
• Degree in Occupational Therapy required
• Registration with the College of Occupational Therapists of Ontario
