Nuvance Health, part of Northwell Health, is seeking a Heart Failure Specialist to join our growing Heart and Vascular Institute at Danbury and Norwalk Hospitals in Connecticut. The ideal candidate will be board certified in Cardiology and board certified/eligible in Advanced Heart Failure & Transplant Cardiology and looking to collaborate with our HF specialists across our larger hospital system. Post fellowship training experience preferred. Leadership experience a plus.
Specific responsibilities include:
* Collaborate with the larger Northwell HF Team and have opportunity to spend time at the main campus
* In and outpatient consultation for Danbury and Norwalk patient populations
* Management of patients on mechanical support in inpatient and outpatient settings
* Continued management of readmission reduction and HF care pathways
