Overview
The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals.
+ 5 x 8 hour shifts (9:30AM to 5:30PM) - Rotating Weekends and Holidays
+ Great Healthcare benefits, and education reimbursement of $5,250 annually
+ Additional compensation for visits over your caseload
This position will travel between Catholic Health's Skilled Nursing Facilities

Our Home Care Transition Nurses :
+ Establish and maintain professional relationships with key physician groups and referral sources.
+ Act as Liaison to referral sources on behalf of Catholic Home Care.
+ Identify potential market growth opportunities and program expansion.
+ Serve as a Customer service representative and act in a professional and supportive manner at all times.
Requirements:
+ Registered Nurse with current licensure in NYS
+ 2 or more years...
