Integrated Care Program Supervisor role for Registered Nurses (RN) or Registered Practical Nurses (RPN) based in Mississauga and surrounding area. Responsibilities
- Lead an integrated palliative care team within a designated neighbourhood, managing day-to-day coordination of services to ensure continuity of care, seamless processes, and optimal patient and employee experience.
- Collaborate with your team to assess, plan, and coordinate with patients, caregivers, and other care-team members to establish, review, and revise care plans that support patients to die in their preferred place of death, respecting patient wishes for MAID as requested.
