This role blends adult preventive medicine and integrated behavioral health, enhancing access to high-quality care for generally healthy adults.
The APP will provide routine wellness and preventive services, early disease detection, lifestyle counseling, chronic disease prevention, and management of common conditions. The role supports population health goals, preventive care metrics, and integrated care pathways across the clinic.
