Job Summary
Public Health Nurse (2) in the Healthy Families Division of HBHC will work with high‑risk families of all backgrounds to promote optimal emotional, physical, and developmental health from prenatal to school transition through assessment, counselling, preventive health teaching, and service coordination in a home visiting model.
Responsibilities
- Access high‑quality data, research findings, and other sources to plan evidence‑informed programs and interventions.
- Apply population health assessment data to identify concerns, trends, and priorities affecting client health outcomes.
- Conduct comprehensive assessments of assets and needs in collaboration with clients.
- Develop mutually agreed plans and priorities for care with clients, considering health equity, trauma and violence‑informed care, and cultural safety.
