Job Description
Our Client, a Behavioral Health company, is looking for a LCSW Behavioral Health for their Fort Wayne, IN location. Responsibilities:
+ Complete psychosocial assessments and initial discharge plans Develop, implement, and update treatment plans Provide individual, group, and family therapy Facilitate psychoeducational groups and family meetings Maintain accurate and timely clinical documentation Participate in multidisciplinary treatment planning Provide crisis intervention as needed Coordinate aftercare planning and community resource referrals Manage assigned caseload Qualifications Master’s degree in Social Work, Counseling, or Psychology (required) Minimum 1 year of therapist experience (all age groups preferred)
