Overview
The Complex Care Support Team (CCST) is seeking a skilled and compassionate Nurse Practitioner (NP) with expertise in geriatrics and dementia care to join our North Zone team. CCST is a specialized consultation service supporting some of the most complex adult patients across the North Zone, including individuals with dementia, brain injury, and complex medical needs. In this role, you will provide comprehensive, patient-centered care across a variety of settings, working collaboratively within an interdisciplinary team. You will bring strong clinical knowledge in responsive behaviours and experience in the assessment and management of dementia and delirium, including medication optimization. You will support patients, families, and care providers by applying best practices and delivering thoughtful, evidence-informed care.
