Penn Home Provider Program is seeking a full-time Nurse Practitioner to join their growing team! The Nurse Practitioner will be cross-trained to provide Palliative Care and Primary Care services to adult patients in the home setting seeing a combination of palliative care consults in the home, as well as primary and transitional care.
