Schedule: Monday Friday, 8:00 AM   4:30 PM clinic hours
Job Setting: Outpatient
Type of Cases: Comprehensive primary care   evaluation and treatment, chronic disease management, care coordination
Credentialing: 60 - 90 days 
Minimum Requirements for Consideration  
Board Certified/ Board Eligible Family Medicine
ACLS, BLS required
Licensure: Any unrestricted state medical license and DEA
