This in office, casual position supports WellFort’s Homeless Health Program, enhancing healthcare accessibility for vulnerable individuals.
Responsibilities
- Deliver primary health care in various outreach activities
- Work alongside Nurse Practitioners and support staff
- Promote health and wellness within the community
- Help manage chronic conditions and medical needs
- Participate in program evaluation and development
Qualifications
- Registered nursing diploma or degree from an accredited program
- Must be registered with the College of Nurses of Ontario
- Experience in community health and social services
- Exceptional interpersonal and communication skills
