LOCATION: Strathmore AB, with travel to surrounding areas
The Primary Care Registered Nurse (PCRN) will work as a part of a multidisciplinary primary care team including allied health professionals and community partners to deliver wholistic patient centered care, with consideration of all social determinants of health. The PCRN provides comprehensive team-based care to patients across the lifespan, in the patients’ medical home and at Calgary Rural PCN central clinic locations. The PCRN may support a variety of programs and services, while exercising a high degree of critical thinking, in an autonomous but collaborative and supportive environment.
