Registered Nurse – CCDC 02/25 (Sioux Lookout)

Overview

Job Summary: Under the general direction of the Manager of MHAP, RAAM, ACT, SLDP, CCDC and Telemedicine, the Registered Nurse provides enhanced support to patients registered to the Centre for Complex Diabetes Care (CCDC). The CCDC RN plans, develops, implements and evaluates diabetes education services, diabetes monitoring, foot and wound care. The CCDC RN will work collaboratively with all members of the interdisciplinary health care team at Sioux Lookout Meno Ya Win Health Centre (SLMHC) and partner agencies to support and enhance the services of the CCDC, and ensure the provision of protected, wholistic, patient centered care to our clients.

Responsibilities

• Provide diabetes education to patients registered to the CCDC and inpatient unit, including medication management, lifestyle modification, and establishing self-management goals.
