Responsibilities
Provide a range of primary care services to the patients of the Centre de santé Univi Health Centre (CSUHC); perform client assessment, follow‑ups, triage, screening, and counselling.
Collaborate with nurse practitioners, physicians and other healthcare professionals to develop and implement individualized care plans.
Participate in the development, delivery and evaluation of educational and health promotion programs.
Maintain accurate and timely records and encounter data in patient electronic medical record (EMR).
Participate in chart and case review.
Communicate information clearly and concisely to patients and health‑care providers on a regular basis.
Educate patients and their families about healthcare, treatments, and procedures.
Participate in the assessment and reassessment of patient care needs.
Adhere to policies and procedures as well as professional standards.
