Summary
As an HD Nurse, you'll be at the forefront of patient care, working collaboratively within an interdisciplinary team to ensure patients/residents receive comprehensive support. Your responsibilities will include:
- Assess patients' physical, social, spiritual, and psychological needs
- Develop, implement, and evaluate individualized care plans
- Incorporate teaching into care plans to educate patients and families
- Make referrals to resources as needed
- Support fellow healthcare professionals
