The home health RN and LPN is responsible for providing care in accordance with the patient's plan of care to include comprehensive health, monitoring of the patient's condition, health promotion, teaching and training activities, and direct nursing care. The ideal candidate must have experience and knowledge of Medicare, excellent communication skills, be able to work independently, and be computer proficient.
