Part‑time Registered Practical Nurse (RPN) – Hospital Partnership Program, North York & Scarborough.
Program Summary
VHA Home Health Care (VHA) is a leading not‑for‑profit provider of home care services in Ontario. RPNs provide and coordinate home health nursing services to diverse individuals, families, and communities in accordance with the College of Nurses of Ontario Practice Standards and VHA policies.
Schedule and Location
Shifts: Days (0900‑1300) and evenings (1300‑2100) with bi‑weekly weekend rotation. Position based in the North York or Scarborough region.
Key Responsibilities
- Assess, monitor, and develop care plans and coordinate care.
- Evaluate care plan interventions through ongoing reassessment and evaluation.
