Job Description
Become an Occupational Therapist with the Transitions Home Program at Providence Care Hospital. This position supports community-based rehabilitation for clients after hospital discharge in southeastern Ontario.
As a temporary full-time therapist, you will assess client needs and develop individualized treatment plans that foster greater independence at home. Collaborating with an inter-professional team, you will be essential to program development and quality care. Additionally, this role includes opportunities for mentorship and education with Queen’s University.
Key Responsibilities:
• Implement treatment plans based on the best rehabilitation practices
• Monitor client progress and adapt care as needed
• Collaborate with clients to set realistic treatment goals
• Supervise and support staff overseeing client care
• Maintain the standards set by the College of Occupational Therapists
Requirements:
• Degree from an accredited Occupational Therapy program
